How to: Restore the default settings to a toolbar.
Solution:
Select the 'View' menu and select 'Toolbars...', select the toolbar and choose 'Reset...'.
1) Select the 'View' menu and select 'Toolbars...'. (The Toolbars dialog box appears.)
2) Select the toolbar to restore. (The selected toolbar is highlighted.)
3) Click 'Reset...'.
4) (Optional) Clear the 'Large Buttons' check box.
5) Click 'OK'.
